Donor Registration Form
Eligibility for Blood Donation:
- Age: Must be between 18 to 65 years old.
- Weight: Should weigh a minimum of 50 kg.
- Health: Must be in good health with no chronic illnesses or infections.
- Tattoos: Should not have gotten a tattoo or piercing in the last 6 months.
- Surgeries: Should not have undergone major surgeries in the past 6 months.
- Pregnancy: Pregnant and breastfeeding women are not eligible to donate.
- Lifestyle Choices: Certain lifestyle choices, such as engaging in high-risk behaviors like intravenous drug.
- Medical History: Should not have a history of diseases such as HIV/AIDS, hepatitis, or malaria.
- Recent Infections: Should not have had any infections such as cold, flu, or fever in the past 2 weeks.
- Blood Pressure: Blood pressure should be within acceptable limits for donation.
- Heart Conditions: Should not have any serious heart conditions that may affect donation safety.
- Alcohol and Drugs: Should not have consumed alcohol or recreational drugs within the last 48 hours.
- Iron Levels: Hemoglobin levels should be within the acceptable range for donation.
- Blood Disorders: Should not have any blood disorders or clotting disorders.
- Vaccinations: Should not have received certain vaccinations within the specified time frame.
- Recent Surgery: Should not have undergone any surgery in the past month.
- Health Check: Must have passed a recent health check-up within the last 6 months.
